We are proud to support Empowerment, a charity with multiple health projects across Blackpool and the Fylde Coast, supported by an employed team of individuals with lived experience of multiple disadvantage
The service partnership collaborates with individuals who occupy a bed in one of the mental health care team wards across the North West. The primary goal is to facilitate their access to housing upon safe discharge. The case management support provided by the partnership includes several components.
First, there is in-ward support, which involves calls and online sessions. Additionally, the partnership supports individuals with their community rehabilitation. Each person receives tailored support based on their specific needs, ensuring they receive the right assistance at the right time. The partnership also works closely with wraparound support services to better understand the health and wellbeing needs of service users. Engaging individuals to make informed decisions about their care is a key aspect.
Furthermore, the partnership considers personal strengths that can be built upon, aiming for better health and social outcomes. It fosters confidence, emotional independence, and promotes self-care. The partnership offers personalised opportunities for stability by developing a range of interdependent community services and effectively linking service users. Intensive one-on-one support empowers users to engage with services effectively.
Additionally, the partnership liaises with local agencies and health professionals, extending the spectrum of opportunities available across Lancashire & South Cumbria. Through signposting and facilitation, the partnership ensures access to recovery activities, social prescribing, and other local initiatives that may benefit the service user. Lastly, it actively promotes engagement with partner agencies, including mental health services, primary care, domestic violence services, and other relevant organisations.
"The support offered by Spring North Bridge to Home project has been very useful in identifying placements for patients who are threatened with homelessness upon discharge from hospital. The case workers have a wide range of knowledge of support available in the local areas which helps inform our discharge pathways. The case workers are proactive in making referrals and helping progress towards discharge.”
