Funded through the UEC Capacity Grant, this project will focus on reducing avoidable hospital admissions by providing non-clinical, wraparound support to people of all ages across Lancaster, Morecambe and Carnforth. The aim is to help individuals stay safe, well and connected in their communities, while easing pressure on urgent and emergency services.
Organisations will work alongside NHS and community partners including Integrated Care Communities, Urgent Community Response teams, Primary Care Networks and the Royal Lancaster Infirmary. The model will support early intervention, improve system navigation and strengthen the links between clinical services and community assets.

Project period: September 2025 to March 2026

This opportunity is open to organisations with experience of delivering place-based, person-centred support, particularly those embedded in neighbourhoods and working with people facing multiple disadvantages. We are especially interested in approaches that are proactive, inclusive and shaped by local insight.
The project will also involve volunteer recruitment, multi-agency coordination and targeted work with people who frequently access emergency care or are at risk of becoming high-intensity users.
At the end of the programme, participating organisations will work with Spring North to share learning, impact and recommendations for future care models.
