Church on the Street – CMHT
Through the CMHT Grant Fund, we are proud to support Church on the Street, a Christian organisation and registered Charity designed to support the most vulnerable members of the community.
72 committed champions deliver nationally approved messaging in relation to Type 2 diabetes in their communities and networks across Pennine and Central Lancashire. They predominantly target families and young people who are at a higher risk of developing, have a pre-diabetes diagnosis or a diagnosis of Type 2 diabetes.
They work within families, community networks and groups to increase knowledge and understanding of Type 2 diabetes and changes they can make in their lives that would have a big impact on their health. They explore and help tackle local barriers and challenges to enabling change in a families or young person life such as exercise and activities. They offer support from in the community in the family’s surroundings and language rather than from a clinical/health perspective.
As well as champions we have two dedicated Family Practitioners, who provide a bespoke 8-week support package to a family or YP. These families and young people are referred by specialised nurses and clinics, so they can get wrap around help and support, not only in a health care setting but in their communities.

Through the CMHT Grant Fund, we are proud to support Stable Lives, a distinguished charity that has made significant strides in mental health support across Lancashire for over a decade.
This initiative is designed with the specific objective of increasing vaccination uptake, particularly first-time vaccinations, among key at-risk groups across Lancashire and South Cumbria. These include individuals suffering from asthma, diabetes, respiratory viral infections (RVI), heart issues, and obesity.
